Confined space entry kills more rescuers than entrants, because the second person goes in without testing the atmosphere that stopped the first. The permit exists to force a gas test and a rescue plan before anyone climbs in, and a digital permit makes that sequence non-negotiable.
What the permit covers
Entry into tanks, vessels, pits, sewers, silos, ducts and any other space with restricted access and inadequate natural ventilation. The defining risk is not the space itself but the atmosphere inside it, which can change while work is in progress.

Breathing apparatus and harness as standard equipment
Required PPE is part of the permit type: hard hat, full body harness, self-contained breathing apparatus, coverall, safety shoes and gloves. The harness matters as much as the SCBA, because it is what allows retrieval without a second entry.
Atmosphere verification, then continuous monitoring
Confined space entry runs on the high-risk workflow. An authorised gas tester signs the atmosphere verification within a two-hour window, and periodic monitoring continues while the entry is live rather than being a one-time check at the start.
That distinction is the whole point. Atmospheres change — hot work nearby, residue disturbed by movement, a valve passing. A single reading at eight in the morning tells you very little about eleven.
Isolation before entry
Where the space connects to live systems, the authorised isolator role executes and signs the isolation. Keeping isolation as a distinct role, rather than folding it into the issuer, means the person who physically locked the valve is the person on record.

Does the permit force an atmosphere test before entry?
Yes. Confined space entry uses the high-risk workflow, where gas test and atmosphere verification is a mandatory signed stage by an authorised gas tester before the permit can be issued.
Is monitoring continuous or only at the start?
The workflow includes a periodic monitoring stage that runs while the work is live, assigned to the Safety or EHS officer, in addition to the initial verification.
How is energy isolation handled?
A separate authorised isolator role executes and signs isolation. It is deliberately not merged with the issuer role, so the record shows who physically performed the isolation.
